Information Text Within the Scope of the Personal Data Protection Law

In accordance with the provisions of the Personal Data Protection Law No. 6698 ( KVKK ) and other legislation; Sara Yaman is providing you with certain information within the scope of this Information Text in order to collect and process your personal data and some of your private data that you have provided to us while benefiting from our health services and other services in accordance with the law.

Sara Yaman, who has the title of Data Controller, may record, store, update your personal data as explained in this Disclosure Statement, share, transfer, classify and process your personal data in the ways listed in the KVKK and relevant legislation, in cases where the legislation or agreements permit and/or make it mandatory, with third parties (relatives of the service recipient and/or public and/or private law legal entities of which the service recipient is an employee/insured, other public and/or private law legal entities, third parties from whom our office receives services or cooperates in order to carry out its activities subject to the purposes specified in the legislation to which it is subject).

Your personal data is obtained by Sara Yaman for the purposes of protecting public health, carrying out preventive medicine, medical diagnosis, treatment and care services, planning and managing health services and their financing; through verbal, written, visual or electronic media, call center, website, verbal, written and similar channels.

Your Personal Data Processed and the Purposes of Processing Them

Your personal data and special personal data, including your health data, may be processed by Sara Yaman in a limited and proportionate manner in connection with the purposes specified in this article, including but not limited to the following   :

Identity Information  : Your name, surname, Turkish identity number, passport number or temporary Turkish identity number, place and date of birth, marital status, gender, insurance or patient protocol number and other identity data that can identify you;

Contact Information : Your address, telephone number, e-mail address and other contact data, as well as personal data obtained when you contact us via e-mail, letter or other means;

Accounting Information  : Your financial data such as your bank account number, IBAN number, credit card information, billing information; your data regarding private health insurance for the financing and planning of health services and your Social Security Institution data;  Health Information : Your laboratory results, test results, examination data, appointment information, prescription information, including but not limited to all health-related personal data obtained during or as a result of medical diagnosis, treatment and care services, and other personal data including your CV provided in this regard if you apply for a job at our clinic, and all personal data regarding your employment contract if you are an employee or related employee of our clinic.

Transfer of Your Personal Data

Your personal data belonging to our service areas are not transferred to any real or legal person (except for the requests of official institutions such as the Ministry of Health and its sub-units, public or private health institutions specified in the Law, Social Security Institution, General Directorate of Security and other law enforcement forces, General Directorate of Population, Turkish Pharmacists Association, Courts and all kinds of judicial authorities).

Method and Legal Reasons for Collecting and Processing Your Personal Data

Your personal data is obtained in all kinds of verbal, written, visual or electronic media, for the purposes stated above and to provide health services within the determined legal framework and to enable Sara Yaman to fully and properly fulfill its contractual and legal obligations within this scope.

Data Owners’ Rights Under the Law

You can contact Sara Yaman’s office at any time to request your personal data;

– Learn whether your data has been processed, the purpose of processing and whether it has been used in accordance with its purpose, and if it has been processed, request information about it,

– You can learn about the third parties with whom your information is shared, both domestically and abroad, in accordance with the law.

– If you think your information is incomplete or processed incorrectly, you may request correction,

– You may request the deletion or destruction of your information within the framework of the conditions stipulated in Article 7 of the Law,

– You may request that your requests specified in clauses (c) and (d) be notified to third parties to whom your information is transferred and that they perform the same procedures,

– You may object to a result that is detrimental to you due to the analysis of your information by automated systems, or

– If you think that your data has been recorded or used illegally and you have suffered damages as a result, you can request compensation for the damages.

If your application for these purposes requires an additional cost, you will be required to pay the fee determined by the Communiqué on the Procedures and Principles of Application to the Data Controller issued by the Personal Data Protection Board. If a written response is given to your application, no fee will be charged for the first 10 (ten) pages, and a 1 TL processing fee will be charged for each page over 10 (ten). If the response to your application is given on a recording medium such as a CD or flash memory, a fee equal to the cost of the recording medium will be charged to you.

The requests included in your application will be finalized as soon as possible and within 30 (thirty) days at the latest, depending on the nature of the request. In order to benefit from your rights under the law, you can submit your applications in writing in Turkish to Sara Yaman’s Clinic, and you can visit the Personal Data Protection Authority’s website for detailed information.

Your applications;

– After filling out the form at https://www.sarayaman.com.tr/en/ , a signed copy should be delivered personally or via a notary to [Bahçelievler Mahallesi Yanyol D-100 Sk. No:14.B Metroport Rezidance, Bahçelievler/İstanbul]

– You can do this by filling out the form at https://www.sarayaman.com.tr/en/ and signing it with your secure electronic signature or mobile signature within the scope of the Electronic Signature Law No. 5070, and sending the secure electronically signed form via registered electronic mail or your previously notified electronic mail address that is registered in our system.

In the application you will make to exercise your rights as the personal data owner and specified above, and which includes your explanations regarding the right you request to exercise; the matter you request must be clear and understandable, the matter you request must be related to you or, if you are acting on behalf of someone else, you will need to present your special power of attorney approved by a notary public.

The Practice reserves the right to make changes to this disclosure text at any time for reasons arising from the Law, secondary regulations and Board decisions. Any changes to be made to the disclosure text and the updated text will become effective immediately as of the date they are notified to you.
